Health Home services
ICHS patients with Washington Apple Health (Medicaid) may be eligible to participate in the Health Home program. Created under the Affordable Care Act, the program offers services to assist Apple Health members with serious chronic conditions. These conditions include diabetes, asthma, heart disease, cancer, and depression.
Addressing the whole person, not just their symptoms
Health Home care coordinators work with each client to develop an individualized holistic care plan that is guided by the client’s personal goals. There is no cost for clients who participate in the Health Home program. Participation does not change the client’s benefits, or any current healthcare or service providers.
As part of the program, Health Home care coordinators assist clients face-to-face with navigating and integrating their health care and social service needs, improve their overall health and well-being, and manage their chronic conditions. Care coordinators will focus on integrating the following services:
- Medical care
- Mental health and well-being
- Chemical dependency
- Long-term services and supports
- Community and social support
Care coordinators can meet you at an ICHS clinic, in your own home or another place of your choice, such as an office in your community or a coffee shop.
Eligibility
You may be eligible if you:
- Are eligible for full Apple Health (Medicaid) coverage
- Have a chronic condition
- Are at risk for a second chronic condition
