What is a geriatric assessment visit?
This is a visit with a specialist provider to evaluate and treat common problems in aging, such as:
- Changes in memory and thinking
- Falls
- Too many medications
- Multiple hospital visits
- Difficulty with mobility
- Bone health
- Chronic pain
- Weight loss
- Poor quality of life
- Planning for the future
How can I schedule a visit?
Please speak with your primary care doctor about your concerns, and they will place a referral for you. The front desk will help you schedule with Dr. Furies.
What should I expect during a visit?
- Geriatric appointments typically last 40-60 minutes
- You will be asked questions to get to know you, your values, and your health
- There will be tests for your memory, balance, and mood, depending on the issues you are experiencing
- The geriatrician will develop a treatment plan together with your primary care doctor based on what is important to you
- Follow-up visits may be necessary to complete the assessment and to provide comprehensive counseling and recommendations
How do I prepare for a geriatric assessment visit?
- Please come with someone who is involved in your care, such as an adult child, grandchild, or caregiver. They can provide critical information about your function that is necessary for diagnosis and play an essential role in your wellbeing.
- Bring all your medications, as well as the system you use to manage your medications (pill bottles, pillbox, or medisets)
- Please complete the paperwork given to you by the front desk before the visit to make your time with the specialist more productive for you
- Since we must accomplish a large amount during the visit, please come 15 minutes before your appointment time
