Behavioral Health Care Coordinator - PACE

Job Summary
The Behavioral Health Care Coordinator functions as a member of the Program of All-Inclusive Care for the Elderly (PACE) collaborative care team that includes the patient, the interdisciplinary team (IDT), and other medical staff. This is a non-clinical position that does not provide therapy or counseling. This position is responsible for providing integrated behavioral health care, including case management, within a PACE care setting, which includes planning, organizing, and coordinating behavioral health services in conjunction with other members of the patient’s care team and identifying external providers who impact patient care.

Position Requirements
- Bachelor’s degree in a mental health field (social work, psychology, counseling, or related field).
- One (1) year experience working with individuals with co-occurring mental health and/or substance use disorders; individuals with a disability; individuals of a vulnerable population (elderly, mothers, children) and/or other high risk groups preferred.
- Suicide assessment, treatment, and management training required by WA State Department of Health within sixty (60) days of hire.

Job Summary
The Behavioral Health Manager is responsible for the planning, organizing, direction, coordination, growth, and evaluation of the Behavioral Health Outpatient Services. This position requires an advanced level of clinical knowledge, communication, systems-based practice and interdisciplinary collaboration (e.g. Administration, Nursing, Medical, Pharmacy, Operations and External Stakeholders) to assure safe, effective, quality patient care.

- Graduation from an accredited Nurse Practitioner program.
- One (1) year experience in community health center primary care or community mental health settings required.
- Active WA State MD, or DO, or ARNP license, and ANCC or AANP Board certification if applicable. Active DEA license and BLS certification. Bilingual preferred – Fluent in API or Spanish preferred.
