Scheduling an Appointment 

You can make an appointment by calling your clinic:

You may also come to the clinic during business hours.

Urgent Care

If you have a medical problem that needs attention right away, please call our clinics at any time of the day.  A nurse will be paged immediately to help you. 

The nurse will ask about your medical problem and consult with a doctor to decide if you need to:

  • See a doctor immediately,
  • Go to the emergency room, or
  • Stay at home and follow the nurse's advice.

Cancelling Appointments

It is important that you notify us when you are unable to keep your appointment.  If you need to cancel or reschedule an appointment, please call us at least one day before your scheduled appointment time.  This will allow us to offer your appointment slot to another patient.  It will also give us enough time to cancel interpreter services we may have arranged for you.

What to Bring

When you come to your appointment, it is important to always bring the following:

  • Appointment card, your insurance card, and/or medical coupon
  • Co-payment
  • Proof of income (if you wish to apply for the sliding-scale fees)
  • Medication bottles or a list of your medications and their dosages
  • List of supplements or herbs your are taking and their dosages.
  • Immunization records for children's visits
  • Signed permission slip from a patient or guardian if a child is accompanied by someone other than their parent or legal guardian

Information for Patients Under Age 18

If you are under the age of 18 and need services such as family planning or sexually transmitted disease (STD) diagnosis and treatment, you are not required to obtain parental permission to receive care.

For all other services, patients under the age of 18 must have permission for their parent or legal guardian to receive treatment.
